In order to reserve a space prior to the start of the academic year, all registration forms must be completed in full and signed in the appropriate places.

The K&M Center’s Standard Rates (current as of September 15, 2014)

 Payments & Refund Policy for Educational Therapy & Academic Coaching Sessions: The hourly session rate for my student is plus any travel fees if the educational specialist or academic coach is required to travel to the session or meeting. Travel fees are charged per session and are calculated based on the distance from the meeting location to the K&M Center Office (1454Cloverfield Blvd #210, Santa Monica, CA 90404). All rates are subject to change. In the event of a change, the new rate will be disclosed to the family prior to sessions to which the rate change applies.

Student allowed one cancellation per month at no charge throughout the academic year. Any additional cancellations within the same month will be charged at full fee. For sessions involving travel the home or school, a minimum of 1 hour notice is required for all cancellations. Any cancellation of session involving travel with no notice or less than 1 hour notice will result in a full fee charge. 

An interest of 1.5% per month will be applied to all balances past-due 30 days or greater. The   to avoid interest charges is to make sure your payment information on file is always kept current. The K&M Center may pause or cancel services and the student may lose his or her reserved spot with their educational specialist/academic coach if payment becomes past due

Student's parents and/or legal guardians acknowledge that no warranty or guarantee, express or implied, is made as to any student's success or performance on any test or in any other procedure or process designed to assess a student's intelligence, knowledge and/or academic abilities and hereby waives any such warranty or guarantee existing by operation of law. Student's parents and/or legal guardians further acknowledge that a student's failure to perform on any test or in any other procedure or process designed to assess a student's intelligence, knowledge and/or academic abilities at the level expected shall not constitute a breach of this contract nor any other legally cognizable cause of action against the K & M Center, Inc., or any of its officers, directors, employees, independent contractors or agents. Student's parents and/or legal guardians hereby release and discharge The K & M Center, Inc. its officers, directors, employees, independent contractors and agents from any and all liability for injury and/or damages allegedly arising out of student's failure to perform at any defined level on any such test or other procedure or process designed to assess a student's intelligence, knowledge and/or academic abilities.

 

IN SIGNING THIS AGREEMENT, we understand that we agreeing to the terms and conditions of this document and are undertaking the full and legal financial responsibilityfor promptly paying the student fees. We also agree to inform The K&M Center immediately of any changes in legal custody or of impending difficulties in payment, and to respond promptly.
